SAFETY SIGN-OFF

By completing the subcontractor Safety Orientation Acknowledgement Form below, you attest to the following:

  1. I have viewed and understand the site orientation training program video.

  2. Been provided an opportunity to inquire on safety related provisions that I do not understand.

  3. Provided copies of all personal training certificates and/or licenses required to execute my scope of work. I understand that if I have not provided required training/licensing records, I am responsible for providing these items before I engage in any activities that may require them. 

  4. Reviewed the Green Leaf Construction safety program, and understand that if I do not adhere to the rules of the program, I will be removed from the site.
